Condense mandatory care plans

Condensing the mandatory care plans to about 8 or 9 to make them more manageable and less chance of contradiction through the care plans. See attached document. Bold section is what the care plan could be called and the sections underneath are what the current assessment areas would link in to the care plans. CQC theme is picking up contradiction in care plans. When staff are making changes they don't always make the changes in all care plans. This would eradicate the contractions they and we are finding
