More information in the scheduled post fall check

Currently, the scheduled "post fall check" that the system provides only has the Sleeping State and Happiness as options for input and it doesn't have enough information on it compared to our old paper forms.

Can these sections be added onto this "post fall check" action please?

Pain - 0-10 Slider
Bruising - yes/no with location if yes
Deformity - yes/no with location if yes
Behavioural Change - yes/no
Change in mobility - yes/no
