Tile categories

Morning. When using the tiles to indicate care given the staff will select how happy (or not) someone appears. When we had paper care plans we were encouraged to write appears happy etc. Our council has asked how can we confirm someones state and say that they "are" rather than appears, especially non verbal people. Can these tiles be changed to indicate appears or seems happy etc.
