CAPE Dependency Tool to be added

Dependency Assessment based on CAPE (Clifton Assessment procedure for Elderly)- It would be good if we could complete a dependency assessment for service users that would give us a CAPE score. This assessment is very widely used with other health professionals and therefore the score provided would be useful.
