placing a section in mental capacity assessment to identify who the decision maker is and who the assessment has been done with done

On the mental capacity assessment in the risk assessment section I suggest you put a section that identifies who the decision maker is and who the assessment was done with to make a best interest decision. I.e family, GP, Social worker etc. We cannot make decisons to act in someones best interest without a MDT meeting
