Dentist/Optician widget

Create a widget for Dentist Appointment and also one for Opticians Appointment on the PCS devices. This would help with CQC inspections as they are requesting documentation of both dentist/opticians appointments, and currently we hare having to enter them on the system as "other appointment", and this looks all a bit messy and likely for information to be missed.
