E-Forms to share with relatives / External professionals.

You could implement an E-Form which can be sent / shared with loved ones to complete information such as life history Etc which automatically goes to the care plan and can evidence relative input.

This would be amazing for CQC evidence of involvement of individuals and their loved ones etc.

If this only had a unique number on and no personal information this wouldn't need to include individual details and could link back with the care plan for a staff member then to review and submit if this is appropriate to be added to the care sections.

Additionally there could be feedback forms, external professional recommendation forms which would help with effective information and record keeping.
