Incident forms

Thank you, however this not make sense, as to why the system would alter the flow of events, on the incident form, once the form is completed and saved. And why it requests that notes 6 hour pre and post incident are requested. The number of notes added are based on how many entries are made by carers.. Can you please explain this further. I presumed that we add notes in order to identify, proceeding triggers to incidents, and the follow up actions.
