Change tiles for assisting bowels

Good afternoon,
When we are clincking the tiles for support required "none, little, alot of help needed" when we are assisting with open bowels this reads to a clinican that we have physicaly helped when actually this means we have assited to clean residents and change incontinence producs could this be changable as clinicaly people may flag?
