Contractures risk assessment tool

We currently had a visit from Safeguarding and they requested to have in place a contractures risk assessment. I have done a paper one as seen below. We need a way for this to be included within the risk assessments in order to flag up and review this monthly or if there are changes. It is just a tick box that we use now on paper on admission but we can`t review this on a monthly basis.
